Role summary
This role is for a Generic Therapy Assistant Practitioner within a new Intermediate Care Team, focusing on urgent community response services. The post holder will support Occupational Therapists and Physiotherapists to deliver interventions in patients' homes, aiming to optimize safe and timely discharge from inpatient stays or prevent hospital admissions. The role involves managing a delegated caseload of non-complex patients, undertaking environmental assessments, and fitting assistive equipment. It is a community-based position, contributing to reducing length of stay and improving admission avoidance.
Key responsibilities
- Support registered Occupational Therapists and Physiotherapists in delivering interventions for patients in their own homes and community.
- Carry a delegated caseload of non-complex patients to deliver interventions prescribed by and under supervision of an Occupational Therapist or Physiotherapist.
- Prioritise own work and carry out duties as prescribed by the Occupational Therapists, Physiotherapists, and AHP Lead, working independently within standard operating procedures.
- Work as part of the Intermediate Care Service multidisciplinary team, undertaking clinical and administrative tasks and liaising with key stakeholders.
- Undertake environmental assessment, deliver, and fit assistive equipment in the patient's home.
- Participate in service assessment, audit, and data collection to demonstrate effective outcomes of AHP intervention.
Essential criteria
- Evidence of good general education, English/Mathematics GCSE, or equivalent qualification grade C or above.
- NVQ level 3 in relevant subject or the equivalent level of knowledge and experience plus additional competency-based training
- Detailed knowledge of rehabilitation intervention
- Comprehensive understanding of medical, orthopaedic, surgical, and social related issues
- Experience of working as an NHS band 3 OT, PT or generic assistant or equivalent non-NHS functional assessment/ rehabilitation role
- Experience of developing/delivering education advice to groups
- Experience of team working / working with the public
Desirable criteria
- Experience of community working
- Previous experience within the NHS or LA with patients who have a range of physical, psychological, emotional and/or social needs
- Work within a multi-disciplinary team setting / admission avoidance / falls
- Good verbal and written communication skills
- Good time management skills
- Welsh language speaking/listening skills at Level 3 or above
- Other

Job summary
We are redesigning our community services model and implementing a new Intermediate Care Team and therefore an exciting opportunity has arisen to appoint General Therapy Assistant Practitioners to help deliver an urgent community response services and will play a crucial role in developing the service, providing essential support and care to individuals, with a focus on promoting independence, dignity, and well-being.
The objective is to optimise safe and timely discharge from an inpatient stay for patients that need some form of continuing care or prevent admission with interventions that can be managed at home.
This is an exciting opportunity to become part of the Hospital@home therapy team. Working closely with our multidisciplinary team to provide meaningful, high quality care and interventions for our patients with the service aimed at reducing length of stay, improving admission avoidance and reducing avoidable readmissions through partnership working with community based services. Whilst this post is community based, there may be opportunity to work in clinics, where required.
