Role summary
This role involves supporting the Integrated Discharge Team to ensure safe and effective patient discharges. You will work closely with ward staff, patients, relatives, and community services to proactively plan discharges and prevent delays. The position requires an organised, communicative individual who can prioritise workloads and use initiative. It's an opportunity to contribute to quality improvement in a dynamic environment.
Essential criteria
- NVQ/Diploma Level 4 or Foundation Degree Qualification in Health/Social Care or extensive experience within Hospital Discharge.
- Completion of Care Certificate.
- Good level of education, including English and Maths at GCSE level or equivalent
- Skills, Knowledge & Abilities
- Knowledge of discharge processes and the role of urgent care.
- Computer literacy skills
- Health and safety risk awareness
- Able to work autonomously and use initiative, tact and sensitivity when working with patients with complex needs - seeking advice when needed
- Ability to work unsupervised and prioritise own workload in a pressurised environment
- Experience of working in a healthcare setting, and an understanding of complex discharge
- Experience of working as part of a multi-disciplinary team
- Evidence of commitment to learning (through portfolio)
