Role summary

Job summary

The Community Rehab Service (CRS) Team is seeking an enthusiastic and experienced Lead Care Co-ordinator to support the delivery of high-quality, person-centred care across the community.

Working closely with the Team leads, and multidisciplinary teams, you will play a key role in ensuring safe and effective service delivery. You will lead the coordination of care, oversee staffing deployment, manage rosters, support recruitment and induction, undertake supervision and appraisal processes, and contribute to quality improvement initiatives.

This is an exciting opportunity for someone with strong organisational skills, leadership experience and a passion for improving outcomes for patients. The role involves close collaboration with healthcare professionals, patients, families and partner organisations to ensure care is delivered safely, efficiently and in line with service standards.
