Role summary

This role is for a Community Support Worker within the Community Enhancing Recovery Team (CERT), which provides intensive support to service users to prevent hospital admissions and facilitate rehabilitation in the community. Essential criteria

manager, and Im really pleased youre looking at this opportunity. The

Community Enhancing Recovery Team (CERT) was est in 2023 and offer intensive

support to service users within the Enhanced Pathway. The Main focus of the

service was to assist people who are in longer term hospital back into the

community. The team have now expanded to include people who are being

considered for locked rehabilitation, the aim here is to actively increase

their support in the community & to avoid admission. Working with the care

coordinators, the right candidate will provide additional support for the

person.
