Role summary
This role is for a Band 6 Registered Nurse (Mental Health, Adult or Learning Disability) to join the NHS Continuing Healthcare (CHC) team. As a Nurse Coordinator, you will undertake complex assessments, case manage individuals eligible for NHS Continuing Healthcare, and support the Discharge to Assess model. You will work with individuals, families, and multidisciplinary professionals to ensure robust and person-centred decision making for people with significant and complex health needs. The position is based in Ipswich, Suffolk.
