Key responsibilities
- Provide support to patients with conditions like COPD, Asthma, Bronchiectasis and Interstitial Lung Diseases (ILD) within the community.
- Provide a rapid response service to prevent unnecessary hospital admissions or early supportive discharge from hospital.
- Work as part of the ACERS Acute Respiratory Infections Virtual Ward.
- Use specialist skills to improve patient outcomes within the home environment.
- Work with local GP practices to provide education and a case management approach.
Essential criteria
- NMC or HCPC registration
- Nursing qualification or Physiotherapy degree
- Effective verbal and non verbal communication skills
- IT literate
- Post Graduate NHS experience
- Experience working with patients who have respiratory diseases
- Motivated to further personal and professional development
Desirable criteria
- Evidence of relevant post graduate training
- Physical assessment course
- Experience supervising students or staff
