Role summary
This role involves providing acute nursing assessment and interventions to patients in their own homes, ensuring they receive complex care and support. The Community Staff Nurse will monitor care quality, update records using technology, and work as part of a multi-disciplinary team to support patients and minimise hospital admissions. The position requires adaptability, strong communication, problem-solving skills, and the ability to work flexibly with minimum supervision.
Key responsibilities
- Assess the healthcare needs of patients and families.
- Monitor the quality of care patients are receiving.
- Be professionally accountable for the delivery of care.
- Utilise technology to update patient records.
- Provide acute nursing assessment & interventions in patients' own homes.
- Refer patients onwards as appropriate to community services.
Essential criteria
- NMC registered Adult Nurse with post Registrstion community Nursing experience experience
- Experience working within a community nursing team as a Registered Nurse
- Experience in Venepuncture
- wound management
- Catheter care
- Palliative care
- Uk car driver and owner
Desirable criteria
- Acute medicine or Emergency Care Experience
- Experience using SystemOne Patient Electronic Record
- IT Literate able to demonstrate ability to maintain accurate , contemporaneous patient records whilst mobile working

We are looking for experienced Adult Registered Community Nurses with a passion for high quality nursing care and also experience in either acute medicine or Emergency Care .The post holder will provide acute nursing assessment & interventions in patients' own homes and will ensure that patients are referred onwards as appropriate to community services once their care episode is complete.
