Essential criteria
- Appropriate professional qualification (nursing or allied health professional) that allows registration with the NMC or HCPC
- Professional UK registration e.g. NMC or HCPC
- Degree level award
- Independent non-medical prescribing or pharmacology level 7 profession relevant qualification.
- Holds level 7 Clinical History Taking & Physical Examination
- Holds level 7 Diagnostics and Decision making Able to demonstrate a good command of the English Language both written and verbal
- Computer Literate in both data entry and communicating via IT - emails and report

Job summary
We are seeking an experienced and motivated Registered Nurse or Allied Health Professional to join ourHospital@Home Team within Hampshire and Isle of Wight NHS Foundation Trust based in Lymington. This is an exciting opportunity to work in a nationally recognised service delivering urgent, holistic care to people living with frailty in their own homes.
Although the successful candidate's nominated base in Lymington the expectation is that they will travel across the area covered by the service to meet demand as needed.

Maintaining and continuously updating their existing knowledge, skills and expertise to inform their enhanced clinical judgement
Use existing knowledge and expertise and enhanced levels of clinical judgement to independently undertake complex and holistic assessments
Critically evaluating and analysing complex clinical interventions to make evidence-based decisions on how to manage the delivery of complex clinical care for a defined population, which may include individuals from diverse cultural, social and belief systems.
