Role summary
Join the Community Response Team as a Band 3 Community Support Worker, providing high-quality care to patients in South Warwickshire. This role involves preventing hospital admissions and facilitating safe discharges by supporting patients to remain independent at home. You will work as part of a multidisciplinary team, collaborating with social services, GPs, and community nursing teams. A valid driving licence and access to your own car are essential, as the team operates 7 days a week with varied shifts.
Essential criteria
- Obtained an educational level of GCSE level A - C English and Maths or equivalent
- Minimum NVQ/Diploma Level 2, willing to work to Level 3 or equivalent higher apprenticeship in healthcare
Desirable criteria
- NVQ /Diploma level 3 or equivalent higher apprenticeship in healthcare
- Experience in delivering care in a healthcare or people centred environment
- Experience working as part of a team
- Evidence of competence in care skills
- Able to delegate work appropriately
- Good written, verbal and non-verbal communication skills
- Competent IT skills
- Able to present information, written and orally in an accurate clear and logical manner
- Ability to organise and prioritise workload
- Ability to write accurate and legibly in patient notes
- Evidence of competence in relevant specialist skills / tasks

Our Community Response Team covers South Warwickshire, has bases at Stratford upon Avon and Leamington Spa working across the county supporting patients that have identifiedhealth and socialneedsas a resultofa change in their healthcondition.The serviceis committed toprovidehigh quality care toprovidean immediate response, aiming to preventavoidablehospital admissionsbyprovidingsupportto patients to remain independent within their own homeandfacilitatingsafe discharges home from hospital.The team workstogether to support patients in the community and work closely with social services, GPs, community nursing teams, patients,relativesand carers.
