Job summary
Within the Poole Community Mental Health Team, an opportunity has arisen for a Band 6 Community Mental Health Practitioner to join the service on a full-time basis (37.5 hours per week, Monday to Friday, 9:00am-5:00pm) .
The successful candidate will work as part of a well-established, integrated multidisciplinary team comprising Psychiatrists, Nurses, Psychologists, Employment Specialists, Physical Health Support Workers, Social Workers, Administrative staff, and Support Time Recovery Workers.
This is primarily a care coordination role, with responsibility for supporting individuals with complex mental health needs throughout their recovery journey. The post holder will undertake comprehensive assessment, care planning, risk management, and multi-agency liaison to ensure safe, person-centred care delivery.
