Role summary
This role supports patients in their homes and community settings as part of a dynamic Urgent Care Service. You will provide practical and compassionate support, including personal care, meal preparation, and medication administration. Key duties involve clinical observations, escalating concerns, and providing feedback to senior clinicians. The role also includes triaging referrals, conducting assessments, developing care plans, and coordinating equipment and onward referrals. This position is vital for helping people remain safe and independent at home, preventing hospital admissions and supporting timely discharges.
Essential criteria
- Evidence of Foundation Degree Level study (Level 5) or equivalent (Nursing Associate) or be able to demonstrate significant experience as a Senior Healthcare worker that is relevant to the job description.
- Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C or equivalent in English and Maths)
- Full UK/EU driving licence

- Evidence of experience as a Senior Healthcare worker
- Experience of working as part of a team
- Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E roster / AT-P / Rio)
- Experience of community environment
- Awareness of professional issues and recent developments in the NHS and Primary Care including Clinical Governance, NICE, Essence of Care, National Urgent Care Agenda
- Knowledge of up to date clinical practice
- Understanding of the role and remit of a Band 4 assessor
- Ability to Manage
- Ability to prioritise care delivery in a professional manner, considering [contact details removed] policies and procedures
- Able to support the implementation of local and national agendas for health under the guidance of the Registered Practitioner
