Role summary
As a Registered Nurse in the District Nursing Team, you will deliver skilled, evidence-based care to patients with complex needs in their homes and community settings. You will work independently and with a multi-disciplinary team to assess, plan, and implement personalised care. The role focuses on supporting patient independence and preventing avoidable hospital admissions. This is an opportunity to make a meaningful difference while developing new clinical expertise.
