Role summary
Join the Integrated Community Rehabilitation Team as a Healthcare Support Worker to provide rehabilitation support to adults in their homes and community settings. Essential criteria

Job summary
Join our Integrated Community Rehabilitation Team as a Healthcare Support Worker and make a real difference to people's lives. Working alongside therapists, and Portsmouth City Council colleagues, you will provide rehabilitation support to adults within their own homes and community settings, helping them regain independence, improve their wellbeing, and achieve their personal goals.
You will assist with rehabilitation programmes, support patients with daily activities, monitor progress, and work closely with the multidisciplinary team to deliver high-quality person-centred care. This is a rewarding role requiring excellent communication skills, compassion, flexibility, and a commitment to supporting people to live independently.
