Role summary
Join the Kington & Weobley District Nursing Team, a dedicated group providing planned and unplanned nursing care to patients in their homes across Kington and Weobley. The role involves delivering high-quality care to improve health and wellbeing, supporting patients to remain independent, and preventing hospital admissions. You will work within a close-knit, supportive team, covering shifts over a 7-day period, including weekends. The base is Kington, but travel within the North West Locality is expected.
Key responsibilities
- Provide treatment and support to patients in their own homes.
- Undertake holistic nursing assessments to encourage patient independence.
- Perform wound care, including NPWT.
- Manage palliative care and end-of-life symptoms, providing support for relatives/carers.
- Manage long-term conditions in a community environment.
- Conduct CVAD and drain management, and chemotherapy disconnects.

- Good Communication Skills
- Nursing experience

We are the Kington & Weobley District Nursing Team, a small, dedicated team providing excellent planned and unplanned nursing care to patients in their own homes.
We aim to deliver high quality care to improve health & wellbeing and to support our patients to remain living at home. We pride ourselves in being hard-working, supportive, friendly and flexible. We are part of the North & West Primary Care Network and work closely with partners across the network.
We are based in the market town of Kington nestled within the beautiful County of Herefordshire - covering both Kington and Weobley surgeries in the north-west area of the county. This rural area has many areas of outstanding beauty and is predominantly agricultural with rolling country side for miles.
We're seeking to recruit a band 5 community nurse who shares our passion for caring for people in their own homes, strives towards excellence in community nursing and who has the ability to work well within a close knit team of supportive colleagues.
Our working shifts are 08:00 to 16:00 and 12:00 to 20:00, over a 7 day period, including regular weekends. There will be an expectation to work within the North West Locality, though the base for this job is Kington.
