Role summary

This is an exciting opportunity for a motivated and enthusiastic registered professional to join the Learning Disability Crisis Response and Intensive Support Team (LD CRIST) as a Band 6 Practitioner. Key responsibilities

  • Triage new referrals from various sources, including patients, carers, or professionals.
  • Provide clinical assessment, support, planning, and oversight for patients in the community at risk of admission and/or placement breakdown.
  • Offer training and advice to carers, providers, and health and social care staff, specifically related to individual needs and Positive Behavioural Support Plans.
  • Provide specialist advice and support to help people remain in the community and prevent inpatient admissions.
  • Monitor and support individuals using pathways like the Mental Health Pathway and Positive Behavioural Support Pathway until risks are manageable/reduced.

Job summary

We have an exciting opportunity for a motivated and enthusiastic registered professional to join the Learning Disability Crisis Response and Intensive Support Team (LD CRIST) as a Band 6 Practitioner.

LD CRIST provides a service 7 days a week, 8:00am to 9:00pm, including bank holidays.

The successful candidate will have the opportunity to discuss flexible working arrangements within these hours. Weekend and bank holiday working is undertaken on a rotational basis.

As a Band 6 Practitioner, you will manage your own caseload while working within a specialist multidisciplinary team comprising nurses, healthcare support workers, occupational therapists, speech and language therapists, psychiatry and psychology colleagues, with input from a range of allied health professionals.

The team provides specialist support, clinical assessments, interventions and risk management for people with learning disabilities who are at risk of admission to an inpatient unit or at risk of placement breakdown within the community across Leicester, Leicestershire and Rutland.

Our aim is to support individuals to remain safely within their communities wherever possible.
