Role summary
This role seeks an enthusiastic and compassionate Transition Nurse Specialist to lead and coordinate care for young people moving from children's to adult services. You will manage a specialist caseload, act as a key contact for families and professionals, and provide expert assessment and care coordination. The position involves working across health, social care, education, and community partners to ensure seamless, person-centred transitions. It offers the opportunity to influence service development, improve patient experience, and deliver education and support.
Key responsibilities
- Coordinate complex transition pathways across multiple agencies and services.
- Provide specialist assessment, care planning, and case management.
- Support young people and families to develop confidence, independence, and self-management skills.
- Work collaboratively with multidisciplinary teams to ensure continuity of care.
- Lead service improvement, education, audit, and quality initiatives.
- Promote safeguarding, patient advocacy, and equitable access to services.
Essential criteria
- Registered Nurse (Children's, Adult or Dual Registered) with current NMC registration.
- Degree in Nursing or relevant healthcare subject, or equivalent experience.
- Evidence of continuing professional development relevant to transition care and long-term conditions.
- Significant post-registration experience caring for children and young people with complex health needs and long-term conditions.
- Experience of coordinating complex transition pathways and managing a specialist caseload.
- Experience of working collaboratively across multidisciplinary and multi-agency teams, including health, social care, education and voluntary sector organisations
- Experience of leading or contributing to quality improvement, audit or service development initiatives.
- Experience of providing specialist advice, education, supervision or mentorship to colleagues and students.
- Demonstrable knowledge and experience of safeguarding children, young people and vulnerable adults.
- Comprehensive knowledge of transition planning, long-term conditions and the health needs of adolescents and young adults.
- Knowledge of national legislation, policies and guidance relating to transition, safeguarding, consent, mental capacity, equality and diversity.
- Understanding of clinical governance, quality improvement, audit, risk management and evidence-based practice
- Ability to analyse audit, patient experience and outcome data to improve services and patient care
- Knowledge of children's, adult and community healthcare pathways and available support services.
- Comprehensive knowledge of transition planning, long-term conditions and the health needs of adolescents and young adults.
- Demonstrates excellent clinical assessment, decision-making and problem-solving skills in managing complex cases.
- Ability to work autonomously whilst leading and coordinating complex transition pathways.
- Excellent care coordination and case management skills across organisational and professional boundaries. Ability to build effective relationships and influence practice within multidisciplinary and multi-agency teams.
- Excellent communication, negotiation and interpersonal skills, including communicating highly sensitive or complex information to young people, families and professionals
- Ability to develop and deliver education, training and clinical support to healthcare professionals and partner agencies.
- Excellent organisational, workload management and prioritisation skills, with the ability to manage competing demands effectively.
Desirable criteria
- Evidence of leadership development or quality improvement training.
- Postgraduate qualification in a relevant clinical specialty, leadership or healthcare management
- Practice Assessor/Supervisor qualification or recognised teaching qualification.
- Quality Improvement, Project Management or Change Management qualification.
- Experience working across both children's and adult healthcare services
- Experience of leading service redesign or pathway development.
- Experience of representing services at Trust or system-wide meetings
- Experience managing clinical resources, including the safe use of specialist equipment and consumables
- Knowledge of Integrated Care Systems and national transition programmes
