Role summary
This role involves working as a Physical Healthcare Lead/Non-Medical Prescriber within the Clayfields Health Care Team at a Local Authority Secure Children's Home. The post holder will manage an autonomous caseload, providing high-quality nursing care by assessing, planning, implementing, and evaluating physical health care plans for young people aged 10-17. Key responsibilities include focusing on public health needs, developing a healthy school's programme, and delivering 1:1 and group interventions. The role requires collaboration with various healthcare professionals and care staff to address complex physical and mental health needs.
Key responsibilities
- Deliver a high-quality nursing service to meet the health needs of a defined caseload and contribute to the care of young people with multiple complex needs.
- As a Non-Medical Prescriber, ensure medication governance in line with Trust policies and procedures.
- Work collaboratively with the Clayfields Health care team and Clayfields Care staff.
- Collaborate with the GP, Advanced Nurse Practitioner, pharmacy, dental bus and Trust schools vaccination service.
- Undertake medication reviews and audits.
Essential criteria
- Portfolio of relevant post qualification CPD highly including specialist knowledge of professional interventions and evaluations
- Training in working with children and young people.
- A Registered Nurse with Specialist Practitioner Degree/Diploma in specific areas of practice, ie/ School Nursing, Paediatric Nursing.
- A Non-Medical Prescriber qualification
- Ability to travel between sites
- Driver's licence
- Contribution to on call-rota for physical healthcare care plans for admissions out of hours

- Experience of designing and delivering different learning and development opportunities for staff teams
- Ensures that Clinical Governance/patient safety issues are understood and addressed in practice.
- Experience in developing evaluation / audit tools and resources as a part of multi-professional team
- Experience of leading the clinical and professional development of others including students
- Highly specialist applied clinical knowledge
- Highly specialist knowledge of how mental and physical health contribute to risk assessment and support management
- Applied knowledge of risk assessment within field of clinical practice
- Knowledge of national policy and guidance and impact on local practice

Job summary
To work as a Physical Health Care Lead/Non-Medical Prescriber in the Clayfields Health Care Team in the Local Authority Secure Children's Home. Working with a defined autonomous caseload under supervision of the Clayfields Health Team Operational Leader, visiting General Practitioner and Advance Nurse Practitioner. Providing high-quality nursing care, through assessing, planning, implementing and evaluating physical health care plans for young people. Focusing on the public health needs of the population of young people within Clayfields Secure Unit( ages 10-17) and developing the nursing aspects of the healthy school's programme including vaccinations.
To provide appropriate nursing care for young people with regards to their physical health needs, primarily via face-to-face contact. Providing advice and information to Children and Young People in a secure setting, the Health Care team, Local Authority Care staff and other stakeholders as appropriate and required.

As part of the Clayfields House Health Team providing 1:1 and group interventions and contributing to the delivery of education programmes for children and young people and teaching and training for Clayfields Staff.
