Role summary
This role is for a Band 4 Nursing Associate to join a community-based nursing team in Carlton. You will provide general nursing care, including catheter management, injections, wound care, and palliative care, in patients' homes and residential care homes. The role involves conducting holistic nursing assessments, planning, and evaluating personalised care needs, and working as an integral part of an integrated healthcare team focused on reducing unplanned admissions.
Key responsibilities

Essential criteria
- Registered Nursing Associate on the NMC register
- Educated to a Foundation degree level specific to Nursing Associate practice or equivalent
- Evidence of continued professional development
- Placement or experience within the 4 fields of nursing
- Understand the scope of the role of Nursing Associate in the context of the Virtual Ward Team and the organisation, and how the role contributes to service development
- Understand revalidation
- Effective communicator with patients, carers, and all members of the multidisciplinary team
- Experience of training and educating staff

- Evidence of additional responsibilities/interests e.g link/champion roles
- Skills & Knowledge
- Good interpersonal and communication skills;
- Ability to work well in a team
- Ability to work on own initiative
- Able to recognise own limitations in the context of the Nursing Associate role
- Ability to participate in reflective practice.
- Self-driven for quality outcomes
- Excellent organisation and problem-solving skills
- Experience if working within a team
- Recognise and work within limits of own competence
- IT skills
- Contractual Requirements
- A full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
