Role summary
This role involves working as a Community Nurse within an Integrated Community Team, delivering high-standard skilled nursing care to patients in their homes or clinic settings. You will assess patient needs, develop and evaluate care plans, and provide skilled nursing interventions and advice. The position also requires managing the Community Nursing Team and caseload in the absence of the Team Leader.
Key responsibilities

- Administer and monitor medications consistent with policies and procedures.

- Contribute to the prioritisation of workload, taking responsibility for a delegated caseload of patients.
Essential criteria
- Registered Nurse with NMC
- Degree or Diploma in Nursing or equivalent
- Experience of working without direct supervision
- Experience of working with student learners in practice
- Ability to work on own initiative
- Ability to effectively manage the workload, prioritise and delegate work
Desirable criteria
- Previous experience of community nursing practice
- Evidence of post-registration education & training
- Demonstrable skills in risk assessment and management

Job summary
To work as a member of the Community Nursing Team within an Integrated Community team (ICT) delivering care to patients/ residents within an Integrated Care Team population.
To provide a high standard of skilled nursing care to people in their own homes or clinic settings.
To manage the Community Nursing Team and Nursing Caseload in the absence of the team Leader - District Nurse / Senior Community Nurse (for annual leave, study leave & sickness).
