Role summary
This role involves proactive assessment, monitoring, and management of complex or assertive outreach patients, ensuring care is evidence-based and aligns with practice standards. You will provide clinical leadership, contribute to service evaluation, and conduct specialist assessments. The position requires leading the development of high-quality services, underpinned by evidence-based practices and NICE guidance, in collaboration with senior colleagues and other Advanced Practitioners. You will also supervise lower-banded staff and work collaboratively with partner organisations and multidisciplinary teams to deliver a 7-day, 24-hour service.
Key responsibilities
- Manage and coordinate multi-disciplinary meetings (referral / review / business / handover).
- Take a lead role in clinical service development and review in conjunction with the Clinical Manager.
- Identify aspects of the service needing improvement and implement appropriate changes with the Clinical Manager.
- Provide clinical leadership and professional expertise to the team.
- Work closely with and assist the Clinical Manager with development and implementation of operational / clinical policies.
Essential criteria
- 2 year band 6 post experience
- Detailed understanding and knowledge of MH
- Experience of working in MDT approach
- Experience of working with other system partners
Desirable criteria
- Experience of working with complex cases that include AOT cohort of patients
- Demonstrate practical experience of staff and student management
- Willingness to develop management skills at Band 7 level

Job summary
You will have continuing responsibility for proactive assessment, monitoring and management of complex or assertive outreach cohort of patients and ensuring care is current, evidence based and in line with practice standards. You will provide clinical leadership to the team in evaluation and monitoring of the service, provide specialist assessment as per the service users, and carer's requirement.
You will lead with other senior colleagues the continuing development of a high quality service that is underpinned by evidence based practice and care pathways / packages and NICE guidance and DH guidance; leading on service development and policy in conjunction with the other Advanced Practitioners, senior clinicians, Clinical Manager and Operational Services Manager.
