Role summary

This is an exciting 12-month fixed-term opportunity for a Community Staff Nurse to join the District Nursing team in the Thame/Princes Risborough area. The role involves providing high-quality nursing care to complex patients in their own homes and residential settings, aiming to prevent acute admissions. You will work within a multidisciplinary team, developing autonomous practice in areas like medication administration, wound care, and end-of-life care. A full skills development and preceptorship programme is offered.

Key responsibilities

  • Work within a locality team to meet the needs of complex patients in the community.
  • Administer medication, provide end-of-life care, and manage deteriorating patients.
  • Perform complex and simple wound care, including negative pressure wound therapy and compression therapy.
  • Conduct venepuncture and utilise advanced assessment and clinical nursing skills.
  • Nurse patients with various conditions holistically in their homes and residential care settings.
  • Collaborate closely with social care, GPs, community therapies, and specialised services.

Essential criteria

  • Demonstrate experience post or pre-registration placements in relevant clinical areas: ? Care of Elderly ? Community ? Orthopaedics ? Neurology ? Respiratory ? Palliative Care ? Long term conditions
  • Registered Nurse or student nurse awaiting confirmation of NMC registration
  • Car driver / owner able to travel to other locations within the Trust

Desirable criteria

  • Desirable but not essential knowledge of working with a single assessment, planning and the delivery of care.
  • Experience of electronic clinical systems and technology e.g. RIO
  • Committed to own professional development to enhance practice
  • Post registration education and extended skills e.g. Intravenous drug administration, compression dressings.
