Role summary
Join the South & City Community Nursing Team in Bradford to provide high-quality, person-centred nursing care to patients in their homes. This role involves assessing, planning, delivering, and evaluating care for individuals with diverse health needs, including long-term conditions, complex wound care, and palliative care. You will work within a multidisciplinary team, promoting independence and self-management. The service operates on a seven-day rota, requiring flexibility and extensive travel across the district.

- Teaching / Assessing in clinical practice
- V100 prescribing for nursing or equivalent
- Training
- Willingness to undergo education / training for both practice and service need
- Knowledge & Experience
- Demonstrates ongoing professional development
- Must be able to travel between multiple sites to undertake numerous visits,
- Understanding and/or experience of interagency working and adult protection.

We are seeking a motivated and compassionate Community Staff Nurse to join our friendly and supportive South & City Community Nursing Team within Bradford District Care NHS Foundation Trust.
Working within our Planned Care Service, you will provide high-quality, person-centred nursing care to individuals with a wide range of health needs, including long-term conditions, complex wound care and palliative care. As part of a multidisciplinary team, you will assess, plan, deliver and evaluate care while promoting independence and self-management. The service operates across a seven-day rota to ensure patients receive the support they need when they need it.
