Role summary
This role involves working as a Specialist Community Forensic Worker within a Multidisciplinary Team, supporting service users transitioning from secure care to the community and managing care pathways for those with mental illness diagnoses. Key responsibilities
- Undertake comprehensive mental health assessments for service users with forensic mental health needs.
- Develop, deliver, and review complex treatment interventions and risk management strategies.
- Act as a care coordinator for service users and provide defined interventions.
- Deliver activities to improve carers' ability to support service users.
- Provide expert forensic consultation, advice, supervision, and training to community professionals.
- Maintain accurate health and social care records and participate in supervision.
Essential criteria
- Diploma level/Degree in relevant health/social care profession, eg RMN, OT, Social Worker Current relevant registration with the NMC or HCPC
- Minimum of 2 years post-registration experience gained working in a range of front-line adult mental health services
- Demonstrates a highly developed understanding of the principles of recovery
- Demonstrates experience of assessing risk and developing risk management strategies
- Demonstrates experience of supervising others or relevant professional practice qualification in mentoring/assessing students and learners
- Is able to deliver from a range of possible key therapeutic interventions CBT, Psychosocial interventions, Family Interventions, DBT approaches
- Able to articulate a detailed understanding of the relevant legal frameworks/legislation including CPA process, Mental Health Act , Mental Capacity Act, and Safeguarding
- Highly developed verbal communication skills, able to engage effectively with people at all levels even when a more assertive approach is needed.
- Highly developed written communication skills, with experience of compiling and sorting notes and reports
- Empathy for individuals who have experienced mental health problems and ability to cope with behavior which may be challenging at times.
Desirable criteria
- Evidence of recent further relevant training
- Experience in Forensic mental health and/ or community mental health
- Experience of undertaking carers' assessments

The Specialist Community Forensic Team provides support and intervention to service users transitioning from secure care to the community and provides care pathway management to all secure service users with a mental illness diagnosis.
We also offer specialist advice and guidance to mental health and other relevant services.
The successful applicant will care coordinate a small caseload with support from an expert MDT. You will also have the opportunity to train and carry out the role of social supervisor and to jointly work with recovery teams.
