Role summary
This role is for a patient-focused Registered Nurse or Allied Health Professional to work as a Clinical Triage Coordinator within Care Connexions. You will develop clinical triage skills in a new community service, focusing on timely, joined-up care and hospital avoidance. The role involves clinical triage of referrals and working closely with a multi-disciplinary team to support safe, coordinated care planning. It operates 7 days a week, from 08:00hrs to 20:00hrs.
Key responsibilities
- Develop clinical triage skills within a new and evolving community service.
- Support timely, joined-up care for patients in the community, with a focus on hospital avoidance.
- Ensure referrals are responded to in a timely and effective way, directing patients to the most appropriate service.
- Work closely with a Multi-disciplinary team (Advanced Clinical Practitioners, Nurses, Therapists, Assistant practitioner) to support safe, coordinated care planning.
- Help shape a developing service and make a real difference to patients by supporting timely access to care.
Essential criteria
- Appropriate Professional Registration
- Diploma or Degree in Nursing or Allied health professional/Health related subject.
- Evidence of continuing professional education or development in area of speciality
- Specialised knowledge in discipline to ensure effective and accurate triage and assessment of patients
- Significant experience in working in an MDT environment, liaising with multiple service providers ensuring appropriate care provision and planning
- Post degree learning and experience in relevant area
- Evidence of leadership experience and learning
