Role summary
The Patient Pathway Co-ordinator (PPC) facilitates the smooth running of the patient journey within dedicated speciality clinical areas, ensuring coordinated administrative processes. They assist patients throughout their hospital experience, ensuring timely, efficient, and appropriate care. The PPC is responsible for managing the patient's pathway, focusing on patient and carer experience, and escalating potential breaches of RTT waiting times. They proactively track referrals, plan and book each stage of the pathway, and contribute to data quality and validation.
Key responsibilities
- Oversee the patient journey to ensure scheduling aligns with NHS Constitution principles and target waiting times (e.g., 18 Week RTT), taking action to expedite pathways where necessary.
- Provide administrative support to consultant/clinical teams, including managing diaries/timetables.
- Receive and manage referrals into surgical services, including data entry for patient registration, referrals, appointment information, and discharge.
- Liaise with health professionals regarding clinical decisions on referrals and upload correspondence to EDMS.
- Organise and maintain support for clinical staff to ensure smooth running of the service and proactively identify opportunities to streamline work.
- Manage Medway Clinic diaries and support clinicians by identifying and reporting potential problems like unreceived reports or double bookings.
Essential criteria
- Minimum of 5 GCSEs or equivalent including English and Mathematics
- A-Level / B-Tec or equivalent experience.
- Experience of using a full range of IT/Patient administration systems
- Experience of patient administration work in a healthcare setting
- Experience of handling complaints
- Experience dealing with customer complaints and customer care.
- The ability to prioritise tasks effectively
- Computer literate, with experience of using a keyboard.
- Knowledge of NHS service provision including the pathway of care
- Knowledge of databases, spreadsheets e.g. PAS, Excel or similar applications
- Knowledge of healthcare administrative systems and processes
- Demonstrates excellent interpersonal, verbal and written communication skills.
- Proactive, takes own initiative
- Willing to take ownership and accountability
- Team player with collegial working style and willingness to share responsibility
- Willingness to work flexible hours to meet the needs of the service.
Desirable criteria
- ECDL.
- NVQ level 3 in Business Administration or Customer Care of equivalent.
- One year's experience working in a similar role in a hospital/health care setting.
- Understanding of medical terminology

Provide cross-cover for other Patient Pathway Co-ordinators to ensure a consistent and effective administration service is maintained at all times.
Main duties of the job
The PPC will assist the patient throughout their hospital experience to ensure that care provided is timely, efficient, and appropriate in regards to all members of the healthcare/ multidisciplinary team. The PPC is responsible for ensuring the safe and efficient management of the patient throughout their pathway, ensuring the patients and carer experience is central to every stage of the patient journey, and ensuring potential breaches of RTT (referral to treatment) waiting times targets are avoided or escalated to the relevant manager. The post holder will proactively manage and track the progress of all referrals to ensure each stage of the pathway is fully planned and booked, whilst also working with colleagues throughout the Trust to ensure waiting time targets are achieved. They will be required to assist in the data quality and validation, ensuring all patients have recorded pathway start dates.
