Role summary

This is an exciting opportunity for an experienced Learning Disabilities Case Manager to join the Continuing Healthcare (CHC) Team in North West London. The role involves coordinating complex care packages, undertaking assessments, and advocating for individuals with learning disabilities and complex health needs. Essential criteria

  • Registered RN5 Learning disabilities nurse, level 1 with current NMC registration Educated to degree level.
  • Educated to degree level
  • Experience of establishing & maintaining good working relationships
  • Experience of working across multi agency services.
  • Experience of holistic assessment
  • Awareness of Deprivation of Liberty Safeguards -Experience of completing Mental Capacity Act
  • An awareness of financial management, especially in relation to brokerage and invoicing
  • Experience of completing Mental Capacity Act Assessments
  • Skills & Knowledge
  • Evidence of continuous professional development
  • Knowledge of National Framework for NHS Continuing healthcare and Nursing Funded Care and associated legislation and guidance
  • Proven clinical Leadership Skills
  • Excellent negotiating skills (to promote change & when dealing with resistance to change)
  • Key Attributes
  • Ability to assess patients with complex needs
  • Ability to lead case conferences/MDT meetings
  • Ability to work independently
  • Ability to manage competing priorities
  • Able to manage own workload
  • Able to communicate clearly and effectively both written & verbally
  • Able to deal with challenges and appeals with the support of the Clinical Lead
