Role summary
This is an exciting opportunity for a Band 5 Community Nurse to join the East Area Out of Hours District Nursing Service in Wrexham. The service operates 7 days a week, from 6 PM to 8 AM, covering Wrexham and Flintshire. The role involves providing timely care to patients in the community to prevent hospital admissions and offering advice and support. Successful candidates will be quick-thinking, able to problem-solve, assess, and deliver holistic care in various community settings.

- NMC Registered Nurse
Desirable criteria
- Evidence of post registration study and training eg POVA, Child protection, intermediate life support
- Pre registration evidence in direct nursing care
- Experience of providing holistic nursing care
- Interest in developing nursing skills
- Clear Understanding of clinical governance framework
- Implement evidence based practice
- Skills & Competencies
- Clear understanding of contract of care
- Ability to document details clearly and accurately
- Clear undertanding of consent process
- Basic IT Skills
- Knowledge of incident reporting policy
- Awareness of Policies
- Knowledge of Incident reporting policy

We currently have an exciting opportunity for a suitably qualified and experienced Band 5 nurses to join the East Area Out of Hours District Nursing Service:
Band 5 - various hours
The Out of Hours District Nursing Service cover a large geographical area, which covers Wrexham and Flintshire and other border locations. The team are based in Wrexham. The service is a 7 - day service working between 18.00 - 08.00 each day.
The aim of the service is to provide a timely respond to patients, families and other healthcare professionals to avoid unnecessary hospital admissions, provide advice and support when other help is unavailable.
Main duties of the job
The successful candidates will be quick thinking, with the ability to problem solve, assess and deliver care to patients in any community setting.
Full induction into the roles will be supported, with opportunities for further career development.
