Role summary
This is an exciting opportunity for a registered Band 6 mental health professional to work across primary and secondary care, integrating mental health pathways and enhancing understanding of mental health within primary care. The role involves providing holistic assessments, linking service users to community resources, and facilitating seamless transitions to secondary care when needed. The practitioner will also participate in multidisciplinary meetings within GP practices and contribute to the expansion of the primary care mental health service.
Key responsibilities
- Actively promote better understanding and knowledge of mental health and wellbeing within primary care.
- Provide specialist assessment of service users with mental health problems within the primary care setting.
- Provide effective signposting based on a holistic assessment of an individual's needs.
- Provide evidence-based, time-limited interventions where appropriate and within scope of professional practice.
- Facilitate swift and seamless transition into other mental health services when required.
- Work collaboratively with service users and their families to support shared decision-making and promote recovery.
Essential criteria
- Previous experience of working with and holding a caseload of individuals with serious mental health problems
- Previous experience of working autonomously; assessing, developing care plans and managing risk within a community setting
- Previous experience of working collaboratively within an MDT
- Previous experience of service development, quality improvement and of bringing innovation and new ideas to practice
- Previous experience delivering biopsychosocial mental health interventions
- Previous experience of co ordinating care and signposting to relevant agencies and services
- Registered Professional in either Nursing (Mental Health), Social Work or Occupational Therapy, with current registration with NMC, Social Work England or HCPC.
- Educated to degree level or equivalent experience
- Previous experience of working with and holding a caseload of individuals with serious mental health problems

- Previous experience of working collaboratively within an MDT

Primary Care Mental Health Practitioners - PCN5 Primary Care Network (PCN)
This role is an exciting opportunity for a registered band 6 mental health professional (Nurse, Social Worker or OT) to work in a dynamic way across primary and secondary care to bring together mental health pathways and support the understanding of mental health within primary care. The role will provide holistic assessment of services users linking people into the community and VSCE resources as well as providing seamless transition into secondary care if it is needed. The role will also form part of multidisciplinary meetings held by GP Practices within primary care.
The primary care mental health service is expanding and is a key component to our community mental health transformation. It is envisaged that the role will closely link to the community transformation of mental health and the core team that is being established. The role will link to a specific primary care network and work closely with the practices aligned to that network. The successful candidate will be based in GP sites within the PCN during working hours.
