Role summary
This role is for an Occupational Therapist within the Southwest Community Mental Health Team in Stratford-upon-Avon, working with adults experiencing severe and enduring mental health difficulties. You will provide specialist occupational therapy assessment and intervention, supporting service users to develop and maintain meaningful occupations, routines, and relationships. The role involves undertaking holistic occupational assessments, developing collaborative intervention plans, and delivering evidence-based interventions within community settings. You will work as an integral member of the multidisciplinary team, contributing to assessment, formulation, care planning, risk management, and recovery planning.
Key responsibilities
- Identify barriers affecting individuals' ability to carry out everyday activities and develop practical strategies to address these.
- Manage the formulation of individual occupational therapy goals in partnership with service users and deliver tailored programmes.
- Support individuals to establish sustainable routines, build coping mechanisms, and strengthen self-management skills through graded activities.
- Assess functional abilities, cognition, executive functioning, and sensory needs to identify difficulties affecting daily activities.
- Recommend suitable equipment, adaptations, and environmental changes, liaising with relevant services.
- Assess and manage risks associated with functional activities, self-neglect, vulnerability, and community environments, escalating safeguarding concerns.
Essential criteria
- Degree or equivalent qualification in Occupational Therapy. Current HCPC registration. Relevant post-registration experience working with adults with mental health needs. Experience of occupational assessment, intervention and multidisciplinary working. Understanding of recovery-focused and person-centred practice, with good communication and risk-management skills. Knowledge of the Mental Health Act, Mental Capacity Act, safeguarding, confidentiality, information governance and relevant clinical information systems
