Role summary
An opportunity has arisen for a Band 5/6 Community Mental Health Practitioner to join the Bournemouth East Community Mental Health Team on a part-time basis. This role involves care coordination, supporting individuals with complex mental health needs through assessment, care planning, and risk management. The successful candidate will work within a multidisciplinary team, focusing on recovery-focused and patient-led practice. Regular supervision and professional development opportunities are provided.
Essential criteria
- Registered and valid Mental Health Nurse
- Evidence of recent professional development in an up to date portfolio
- Job Specific
- Able to assess, plan, implement and evaluate programmes of care.
- Risk assessment and management
- The ability to prioritise and meet deadlines; organise work of self and others; supervising, teaching and assessing; self-motivated with the ability to motivate others; effective resource management.
- Skills in managing complex cases.
- Works well in a multi-disciplinary team.
Desirable criteria
- Awareness of Primary Care and Steps to Wellbeing.
- Able to organise and co-ordinate services during the absence of more senior staff.
- Applies current research and theory into practice.
- Extensive post graduate experience in working in community mental health.
- Additional requirements
- Demonstrable skills in written and spoken English, adequate to enable the post holder to carry out the role effectively
- Business travel
- Subject to the provisions of the Disability Discrimination Act, able to travel using own vehicle on Trust business.

Job summary
Within the Bournemouth East Community Mental Health Team, an opportunity has arisen for a Band 5/6 Community Mental Health Practitioner to join the service on a part-time basis (33.75 hours per week, Monday to Friday, 9:00am-5:00pm).
For the right candidate we could consider a developmental role from Band 5 to Band 6.
The successful candidate will work as part of a well-established, integrated multidisciplinary team comprising Psychiatrists, Nurses, Psychologists, Employment Specialists, Physical Health Support Workers, Social Workers, Administrative staff, and Support Time Recovery Workers.
This is primarily a care coordination role, with responsibility for supporting individuals with complex mental health needs throughout their recovery journey. The post holder will undertake comprehensive assessment, care planning, risk management, and multi-agency liaison to ensure safe, person-centred care delivery.
