Role summary
This role is for a Community Out Of Hours Staff Nurse within the Berkshire Out of Hours Community Nursing Team. You will deliver care to patients in their own homes, assessing their needs and implementing nursing care plans. The position involves working overnight shifts, specifically 8 x 4-hour shifts between 7:00 PM and 11:00 PM. The successful candidate will contribute to providing high-quality, evidence-based care and promoting self-care among patients.
Key responsibilities
- Holistically assess the needs of patients and carers, and plan, implement, and evaluate programmes of nursing care.
- Undertake first visits and take immediate action, reporting to the Team Leader/Community Nursing Band 6/Community Matron.
- Undertake practice assessor and supervisor training to support students and new staff.
- Provide care according to the available evidence of best practice and support the development of evidence-based practice.
- Actively promote self-care for patients and integrate Health Promotion into care.
- Demonstrate knowledge and skills in areas such as assessment, medication management, bladder/bowel care, tissue viability, palliative care, and symptom control.
Essential criteria
- Registered General Nurse (RGN)
- NMC Registration
- Evidence of continuing professional development
- Demonstrable experience of excellent communication skills which promote two-way communication and help to establish collaborative team working and patient centred care.
- Uphold the Trust's values of being caring, committed and working as team through demonstrable examples and experience.
- Proficient use of digital tools and applications including Word, Outlook and patient management systems.
- Ability to travel between multiple Trust sites and non-Trust locations, including patient homes, some of which may not be easily accessible via public transport.

The Out of Hours District Nursing team works overnight from 19:00pm to 7:00am. This Post is for 8 x 4hrs 19:00-23:00 Hours a week. This will be different nights and can be discussed at interview.
