Role summary
This role is for an enthusiastic and compassionate Adult Registered Nurse or Paramedic to join the Urgent Community Response Team (UCRT) as a Clinical Lead. The successful candidate will provide senior clinical leadership, undertake clinical duties, and support the team in managing complex patients at home. The UCRT delivers rapid assessment and treatment for patients experiencing health crises in their own homes, aiming to prevent hospital admissions. This position requires expert knowledge in community care and a commitment to high-quality, safe, and effective rapid nursing intervention.
Key responsibilities
- Provide senior clinical leadership within the URCT team.
- Undertake a range of clinical duties and nursing procedures.
- Support the team to manage more complex patients within their own home.
- Perform comprehensive, specialist, holistic assessment for individual patients.
- Organise and manage own time, allocating/delegating tasks appropriately.
Essential criteria
- NMc Pin/HCPC registration
- NMP qualification
- consultation and assessment qualification
- 3 years experience as a band 6
Desirable criteria

- Good communication Skills

Join our Urgent Community Response Team (UCRT) and help deliver high-quality, patient-centred care in the community, supporting people to remain safely in their own homes and avoiding unnecessary hospital admissions. We are looking for a proactive, supportive band 7 nurse/paramedic with proven clinical leadership and decision-making skills.
UCRT is a 7-day service providing rapid assessment and treatment for patients experiencing a health crisis in their own homes, including nursing and residential care homes. Working closely with partners across Mid and South Essex--including ambulance services, acute hospitals, GPs, social care, community services and the voluntary sector--we provide holistic assessments within two hours of referral and deliver timely, coordinated care.
As a clinical lead you will be expected to provide senior clinical leadership within the URCT team and undertake a range of clinical duties and nursing procedures along with supporting the team to manage more complex patients within their own home.
You will need to demonstrate expert knowledge surrounding community care and a commitment to provide high quality, safe and effective
