Role summary
This is an exciting opportunity for an enthusiastic, motivated, and dynamic Discharge Navigator to join a multi-professional team. The post holder will facilitate safe and effective patient discharges from hospital to community settings using a multidisciplinary approach. They will coordinate patient assessments, support discharge plans, and proactively identify and resolve potential delays. - Education to GCSE standard to include English
- KNOWLEDGE AND UNDERSTANDING
- Knowledge of patient administration systems (Cerner)
- Able to communicate effectively with a range of Stakeholders, patients, relatives and carers
- Understanding of how complex organisations work
- Able to work effectively as part of a team but also able to work under own initiative
- Able to take instruction from senior staff

- Knowledge of spreadsheet use
- Previous experience in caring/nursing setting (NVQ Level 3
- Experience in acute hospital environment
- IT Literate
- SKILLS/ABILITIES

An exciting opportunity has arisen for an enthusiastic innovative, motivated and dynamic Discharge Navigator who will be working as part of a multi-professional Team to support the smooth running and co-ordination of discharge services.
The post holder will be responsible for facilitating safe and effective discharges of patients from hospital to a community setting through a multidisciplinary approach. They will coordinate assessments of patients needed and support the discharge plan. The post holder will be proactive in identifying concerns or issues that may delay effective discharges and work with the Integrated Discharge Team to overcome and limit these.
The successful candidate will work closely with all ward teams ensuring appropriate referrals are seen and made to local authorities and preventing unnecessary, avoidable delays for patient discharge.
This role will extend to support and ensure that patients who have been discharged feel supported and have received the support that was requested. You will need to have critical thinking skills and be able to communicate effectively. This role will be full time across 7 days so flexibility will be
