Role summary

Key responsibilities
- Assess, provide, and review nursing care, advice, and information to patients and carers in their home environment or appropriate care setting.
- Maintain patients in their preferred place of care, enabling them to maximise independence and optimise quality of life.
- Work under the direction of the Community Specialist Nurse and Nurse Team Manager.
- Hold a designated caseload and deputise in the absence of the Community Specialist Nurse.
- Provide person-centred care based on holistic nursing assessment to plan, implement, and evaluate nursing care.
- Promote and support self-management, working with people to manage long-term conditions, prevent hospital admission, and support early discharge.
