Role summary
This is an exciting opportunity for a motivated Band 5 Physiotherapist to join the Brent Home First Service. The role involves supporting timely and safe discharge from acute and urgent care settings, helping patients recover and remain independent at home. You will manage your own caseload, provide assessment, treatment, and rehabilitation for patients with complex health needs, and work collaboratively within a multidisciplinary team. The service operates 7 days a week, and full-time staff are expected to participate in a rota covering early, late, and long day shifts.
Essential criteria
- Diploma/Degree in Physiotherapy
- Current AHP with UK HCPC registration
Desirable criteria
- Registration with the Health and Care Professions Council
- Relevant work experience in a variety of clinical placements.
- Evidence of up-to-date CPD oExperience of working as part of an MDT
- Use of functional assessment and activity analysis.
- Evidence of previous employment as an Occupational therapist, apprentice, assistant
- Experience of delegation to support staff
- Skills and Knowledge
- Knowledge of current best practice in PT, demonstrate clinical reasoning, applying theory to practice.
- Ability to work single-handedly with individuals and groups.
- Understanding of professional ethics and their application in practice
- Adherence to national, professional and work-related policy
- Health, safety and risk awareness
- Demonstrates an understanding of clinical governance and its implications for the profession.
- Basic knowledge of health legislation and current practice, risk assessment and management
- Good organisational skills, ability to prioritise, manage time/caseload effectively
- Understanding of team dynamics and good interpersonal skills
- Effective written and oral communication skills. Ability to listen and present information, written and orally in a clear, concise and logical manner.
- Good IT skills (word processing, and internet)

Job summary
An exciting opportunity has arisen for a motivated and enthusiastic Band 5 Physiotherapist to join the Brent Home First Service.
The Home First Service supports timely and safe discharge from acute and urgent care settings, helping patients to recover and remain independent within their own homes. Working as part of a multidisciplinary team, you will provide assessment, treatment, and rehabilitation for patients with a range of complex health and social care needs.
You will manage your own caseload, contribute to clinical decision making, and work closely with acute trusts, primary care, community services, and social care teams to deliver integrated, person centred care and achieve positive outcomes for patients.
This role offers an excellent opportunity to develop your community rehabilitation skills within a supportive team environment, with access to regular supervision, training, and professional development opportunities.
The service operates 7 days a week between 8:00am and 8:00pm, including weekends and bank holidays. Full time staff are expected to participate in a rota covering early, late, and long day shifts as required.
