This role is for a passionate and qualified Psychiatric Nurse to join the South Oxfordshire AMHT in Abingdon. Key responsibilities
- Act as the allocated key worker for a caseload of patients with severe and enduring mental illness.
- Undertake assessment, collaborative care planning, and review of patients' needs as part of a multi-disciplinary team.
- Conduct bio-psycho-social assessments for referred patients, potentially offering brief evidence-based interventions.
- Work in partnership with service users and their families/carers to develop treatment plans.
- Complete duty tasks, including triaging new referrals and identifying appropriate services.
- Complete risk assessments and appropriately escalate any concerns about risk or safeguarding.
Essential criteria
- Professional Qualification/Registration (Degree or equivalent)
- Evidence of Continuing Professional Development (CPD) portfolio
- To have experience of working within a multidisciplinary team
- Evidence and experience of risk assessment and risk management
- To be IT literate
- To have excellent English written communication skills
Desirable criteria
- Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management)
- Experience of working in a community mental health team
