Role summary
This role is for a Community Mental Health Nurse within the Poole Community Mental Health Team for Older Persons. The team provides assessment, treatment, and support for individuals with organic and functional mental health illness. The nurse will manage a defined caseload, act as a Care Coordinator, and provide person-centred care to aid patient wellbeing and recovery. The position is fixed-term for 12 months, with consideration for part-time hours.
Key responsibilities
- Act as a dedicated Care Coordinator for a caseload of patients.
- Provide person-centred, holistic care to assist in aiding patient wellbeing and recovery.
- Manage caseload with support from the team leader and wider team.
- Develop clinical assessment skills and perform risk assessment and management.
- Work collaboratively with carers and other professionals.
Essential criteria
- Registered Mental Nurse
- Valid registration with NMC
- Evidence of continuous professional development
- Valid RMN registration
- Job Specific Experience
- Able to assess, plan, implement and evaluate programmes of care.
- Risk assessment and management
- The ability to prioritise and meet deadlines; organise work of self and others; supervising, teaching and assessing; self-motivated with the ability to motivate others; effective resource management.
- Skills in managing complex cases.
- Works well in a multi-disciplinary team.
- 1 years experience post qualification

- Awareness of Primary Care and Steps to Wellbeing.

Job summary
Poole Community Mental Health Team for Older Persons is recruiting for a Community Mental Health Nurse. This role is essential to maintaining continuity of care and supporting the team in providing care to people experiencing severe mental illness and/or organic mental illness. Consideration will also be given to applicants seeking part-time hours.
The service is a specialist team providing assessment, treatment and support for individuals suffering from organic and functional mental health illness. Experience of working with older adults with organic / functional mental illness in an inpatient or community setting would be an advantage.
The ideal candidate will be enthusiastic, self-motivated and hard-working, with a positive approach to delivering high quality care to the older person and be willing to work collaboratively with carers and other professionals. You will carry a defined caseload and manage this with the support of the team leader and the wider team.
