Role summary

This role is for an experienced and motivated Registered Nurse (RN/RMN) to join a developing community-based Continuing Healthcare, Palliative and End of Life Care service in Carmarthenshire. The post holder will combine community outreach with acute hospital in-reach, focusing on early identification, assessment, care coordination, and discharge planning. You will provide senior clinical oversight, leadership, and support to multidisciplinary teams, aiming to improve patient outcomes and experience in a seven-day service.

Essential criteria

  • Registered Nurse (Part 1) or Registered Mental Health Nurse (RMN)
  • Current NMC Registration
  • Bachelor degree (or equivalent experience)
  • Pertinent postgraduate diploma (or equivalent development/ experience and a willingness to undertake appropriate post graduate diploma level study)
  • Understanding of wider nursing and health care related agenda

Desirable criteria

  • Teaching qualification (or equivalent experience)
  • Recognised management qualification
  • Community Nurse Prescriber
  • Specialist Practice Award in District Nursing
  • Bronze IQT
  • Appropriate experience within community/specialty and able to demonstrate sound knowledge, including experience of caring for people with life-limiting illness and/or cognitive impairment.
  • Able to demonstrate sound knowledge of NMC Code (2015)
  • Experience of working in a multi-professional environment
  • Experience of working collaboratively across acute and community settings to support timely assessment, care planning and coordinated patient flow.
  • Experience of running a ward/area/department
  • Experience of working with preregistration nursing / midwifery students
  • Experience of formal /informal teaching and of mentoring preregistration nursing or midwifery students
  • Experience of coordinating a team
  • Experience of facilitating learning in and from practice
  • Previous experience of leadership and management
  • Awareness of current issues in nursing and specialty
  • Awareness of financial and resource management
  • Language Skills

Job summary

Become a part of a developing service at the forefront of community-based Continuing Healthcare, Palliative and End of Life Care in Carmarthenshire. We are looking for experienced and motivated Registered Nurses (RN/RMN) to undertake a innovative Band 6 Sister/Charge Nurse role that combines community outreach with acute hospital in-reach.

Working across community and acute settings, you will play a pivotal role in early identification, assessment, care coordination, discharge planning and clinical decision-making, helping people receive the right care, in the right place, at the right time.
