We are seeking a highly motivated practitioner with a passion to improve the experience of our ageing and frail population. The post holder must have knowledge and prior experience working within Care Homes or Community Hospitals, management of patients with high clinical frailty scores and undifferentiated complex care presentations. Excellent communication skills are essential to communicate with patients and carers regarding clinical conditions and to lead sensitive discussions around anticipatory care planning and end of life care. As part of integration the role will involve working across organisational boundaries and in partnership with other health and social care teams.
The successful candidate will join a team who focus on the assessment and treatment of our frail and older patients presenting with any acute deterioration in our care homes, intermediate care settings and community hospitals Monday-Friday
