Role summary
This role involves scheduling and allocating care calls using the Complex Discharge Tracker System within the Home First Team. The post holder will work with Locality Teams to ensure referrals are allocated within discharge timescales and the system is validated and updated. Flexibility is required for rotational shifts, including weekends, across North Staffordshire office bases. The Home First Team provides short-term reablement intervention to support independence and meet health and social care needs.
Essential criteria
- GCSEs or equivalent Certificates/Qualifications in Administration
- Ability to effectively maintain, update and sustain systems
- Excellent communication skills
- Demonstrate a commitment to continuous professional development
Desirable criteria
- NVQ Level 3 in Care or equivalent qualification or experience.
- Significant experience of working in health / social care environment
- Experience of working within the constraints of legislation, regulation, policy and procedures
- Experience of delivering improvement to consistently achieve and exceed personal and team objectives
- Experience and ability to work collaboratively
- Experience of work planning & allocation, effective coordinator of allocated care calls to meet the service delivery
- Significant experience of supporting and enabling others to undertake day to day scheduled activities
- Ability to liaise with multi-disciplinary professionals
- Experience of Health and Social Care working
- Experience of working alongside primary care teams including the voluntary and third sector.
- Knowledge, Skills and Attributes

The Home First Team is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands Partnership University NHS Foundation Trust (MPFT).
The service seeks to offer support to people ranging from those that need minimal support for a time limited period to rebuild confidence, learn new skills and relearn skills through to people at the end of their life requiring skilled health & social care interventions. The service operates 7am to 10pm 365 days per year.
The post holder will be responsible for the scheduling and allocating care calls on the Complex Discharge Tracker System. They will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales. They will ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.
The post holder will be expected to commute to the office bases across North Staffordshire, if required.
