Role summary
This role is for a highly motivated and enthusiastic Registered Adult Nurse to work as a Community Staff Nurse within Integrated Care Teams in Croydon. The position focuses on transforming adult community services, promoting independence, and providing well-coordinated care. Responsibilities include promoting health, providing high-standard care, and managing caseloads in the absence of a District Nurse. The role requires a clean UK driving license and access to a car.
Key responsibilities
- Promote the mental, physical, emotional, and social health of individuals, families, and groups.
- Provide a high standard of care to individuals and families.
- Be aware of the needs of patients and their relatives and have knowledge of statutory and voluntary services.
- Be an effective member of a Nursing Team working within the community directorate.
- Be responsible for the caseload management and supervision of the nursing team, in the absence of the District Nurse/Caseload Holder.
Essential criteria
- Registered General Nurse or
- NMC Register part 1 - Adult
- Awareness of current issues and developments in community Care
- Adherence to NMC Code of Conduct.
- Knowledge and understanding of the principles and benefits of Reflective practice
- Able to demonstrate the role of the community staff Nurse
- Good interpersonal and communication skills
- Ability to work as part of an effective team
- Ability to manage own workload
- Able to use own initiative.
- Ability to maintain accurate records
- Knowledge and awareness of principles of Audit and research
- Demonstrate an awareness of the complex issues involved when working across organisations and disciplines.
- Demonstrate knowledge and experience of multidisciplinary working including effective liaison with members of the multidisciplinary team
- Ability to give appropriate accurate feedback
- Ability to work within own limitations
- Valid full UK driving licence
- Access to a car for work purposes
- Ability to work flexible hours including Weekends and Bank Holidays.
Desirable criteria
- Post registration qualification
- Mentorship for Practice course
- Previous experience of community nursing
- IT literate e.g. Word processing and email.
- Knowledge and awareness of process of managing complaints
