Role summary
This is an exciting opportunity for an experienced and motivated Senior Nurse to join the East Lancashire District Nursing Service in a permanent Band 6 role. The successful candidate will support the Clinical Team Leader in coordinating and delivering high-quality patient-centred care in the community, including assessing complex patients and completing Senior Nurse Reviews. The role involves working closely with multidisciplinary teams, supporting students and new staff, and managing staffing rotas. Flexibility to work a rotational shift pattern, including evenings, weekends, and bank holidays, is required.

- Registered Nurse i.e. RN Adult branch
- Community Specialist Practitioner Qualification or willingness to work towards this

- Non-medical prescriber V100/V150/V300
- Leadership training
- Level 6 study to support service delivery and development
- Mentorship or Coaching qualification
- Substantial district nursing experience
- Experience in holistic assessment of complex patients and care coordination
- Experience of multidisciplinary team working
- Experience of caseload management and work allocation
- Experience of service development/supporting change in practice
- Knowledge and Skills
- Ability to use information technology effectively
- Ability to develop effective working relations with other agencies and professionals
