Role summary
This role is for a Community Nursing Associate to join the Wymondham and Attleborough community nursing teams. You will contribute to care packages for patients with long-term conditions, palliative care, and rehabilitation needs, primarily visiting patients in their homes and care homes. The position involves working within an integrated team to facilitate early hospital discharge and prevent unnecessary admissions, collaborating with various healthcare professionals and agencies. You will manage your own daily caseload under supervision, applying competency-based treatments and ensuring accurate documentation.
Key responsibilities
- Prepare for, carry out and monitor assessments and treatments in specified clinical areas, and discharge in line with predetermined department protocols.
- Apply competency based treatment techniques/care to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed treatment/care plan.
- Plan and prioritise own visits to delegated patients, and ensure these are documented in personal work diary.
- Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis.
- Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
- Manage your own daily caseload with supervision and support from the Clinical Lead and the Band 5 & 6 Nurses.
Essential criteria
- Foundation degree programme or equivalent demonstrable knowledge and skills acquired
- Registered Nursing associate on the NMC register
- Experience of working within a health / care setting
- Able to communicate effectively using the English language in both verbal and written forms
- Courteous, respectful and helpful at all times
- Ability to communicate with members of the public and health and care providers.
- Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team
- Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
- Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role may contribute to service development
- Evidence of time management skills and ability to prioritise
- Intermediate IT skills
- Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact
