Role summary
This role involves coordinating care for patients, particularly those with long-term conditions or complex needs, within a GP practice setting. Key duties include communicating with patients and healthcare professionals, managing patient records, and supporting care planning. The successful candidate will work as part of a multidisciplinary team, focusing on patient-centred care and administrative support.
Essential criteria
- Previous experience working in a healthcare, primary care, social care or administrative environment.
- Good understanding of the needs of patients with long-term conditions, complex needs, older people and/or vulnerable patients.
- Excellent communication and interpersonal skills, with the ability to communicate effectively with patients, carers and healthcare professionals.
- Strong organisational and time-management skills, with the ability to prioritise workload effectively.
- Ability to work independently while also contributing effectively as part of a multidisciplinary team.
- Good IT skills and confidence using electronic systems and databases.
- Ability to maintain accurate, confidential and up-to-date patient records.
- Understanding of confidentiality, information governance and data protection requirements.
- Ability to demonstrate empathy, patience and a non-judgemental approach when supporting patients.
- Commitment to equality, diversity and treating patients with dignity and respect.

- NVQ Level 2/3 in Health and Social Care, Business Administration, Care or a related subject.
- Experience working within a GP practice, NHS, community healthcare or social care setting.
- Knowledge of local health, social care and voluntary-sector services.

- Experience working with multidisciplinary teams.
- Knowledge of EMIS Web, SystmOne or other primary-care clinical systems.
