This is an exciting opportunity for a patient-focused Registered Nurse to join the Urgent Community Response Team and Care Connexions. Essential criteria
- Registered Nurse (Adult) with current NMC registration
- Degree in appropriate subject or evidence of education to degree level
- Sound understanding of long-term conditions/ frailty syndrome and management.
- Good understanding of systems in health and social care that are wider than immediate role.
- Good understanding of safeguarding adults and mental capacity
- Evidence of post registration experience working in a community setting
- Proven experience of undertaking complex assessments
- Continuing engagement in professional development and of reflective practice as evidenced in a professional portfolio

We are seeking a motivated and enthusiastic Band 6 Registered Nurse to work across the Urgent Community Response Team and Care Connexions.
