Role summary
This role is for a motivated and compassionate Community Health Practitioner to join the 0-19 Public Health Nursing Service in Worcestershire. You will work with children, young people, and families, delivering high-quality, person-centred care in community settings. The position focuses on promoting independence, prevention, and early intervention, contributing to cross-service delivery and supporting the Healthy Child Programme. You will provide evidence-based support and interventions, including facilitating well-child clinics and responding to advisory calls.
Essential criteria
- NVQ Level 3 in Children's Care, Learning, and Development or equivalent qualification grades A-C) or equivalent, including English and Maths.
- Care Certificate standards within 12 weeks of commencing employment if not already held.
- Safeguarding training and understanding of safeguarding policies and procedures relevant to children and young people.
- Proficiency in using basic Microsoft Office, email (Outlook), and electronic health records for data entry and record management
- Significant experience working with children, young people, and families in a health, social care, or educational setting.
- Experience delivering health promotion activities or interventions, including developmental screening and emotional well-being support.
- Experience working within diverse community settings, such as schools, early years settings, or family support services.
- Practical experience identifying and escalating safeguarding concerns in line with organisational procedures.
- Proven ability to work collaboratively with multi-disciplinary teams and external agencies to deliver integrated care.
- Experience providing advice and guidance to families, maintaining professionalism in sensitive or challenging situations.

- Experience of working with children, young people and families.
- Knowledge of child developmental milestones and common health challenges across the 0-19 age range.
- Understanding of public health issues relating to children and their families.
- Understanding of the importance of teamwork
- Clear understanding of safeguarding responsibilities and confidence in identifying risk.
- Knowledge of the Healthy Child Programme
- Understanding of specific tools such as ASQ developments or school health questionnaires.
- Skills & Abilities
- Engaging with children, young people, and families in a sensitive and empathetic manner.
- Ability to communicate clearly and effectively in English in both verbal and written formats
- Capable of managing a range of priorities and planning own workload effectively.
- Awareness of own limitations.
- Ability to guide and support junior staff, students, or volunteers in health promotion and clinical tasks.
- Experience in escalating complex cases appropriately.
- Additional Criteria
- Willingness and ability to travel across the county when required.
- Willingness to undertake additional training to enhance knowledge and skills.
