Role summary
This Intervention Practitioner role is within the Mental Health Assessment and Intervention Team, providing high-quality care for individuals experiencing mental health problems in Plymouth. The post involves delivering evidence-based recovery-focused mental health interventions across various settings, including GP surgeries and community hubs. You will work collaboratively within a multi-disciplinary team, conducting mental state and risk assessments, and offering brief psychotherapeutic and social interventions. The role requires strong communication and networking skills to support service users, carers, and partner agencies.
Essential criteria
- Excellent communication skills including the ability to engage with users of services experiencing a wide range of complex mental health presentations.
- Ability to form positive therapeutic relationships with users of services, carers and other professional bodies.
- Ability to assess situations, make decisions, co-ordinate and organise appropriate responses.
- Assessment, planning, and evaluation of mental health difficulties.
- Able to risk assess.
- Ability to work without direct supervision.
- Experience in applying psychologically informed interventions.
- Able to demonstrate commitment to high quality care and service provision.
- Able to prioritise work to reflect changing needs.
- Ability to manage own time.
- Ability to work flexibly.
- Standard typing and IT skills.
- Successful completion of Health Screening.
Desirable criteria
- Able to participate and present information in a group setting.
- Have an awareness and ability to maintain personal boundaries.
- Research awareness
- Supervising learners and other staff
- Qualified to foundation degree or equivalent qualification in a relevant subject e.g. Health & Social Care.
- Additional requirements
- Ability to manage change in a positive manner.
- Recognise peoples right to privacy and dignity, treating every person with respect.
- Motivated, reliable and visionary.
- Enhanced DBS check will be required for this role.
- Evidenced based interventions relevant to the service.
- Knowledge of Multi-Disciplinary Working
- Understanding of Recovery principles in Mental Health
- Knowledge and understanding of both adult and childrens safeguarding guidance.
- Knowledge of the Mental Health Act 1987.
- Knowledge of the Mental Capacity Act & Deprivation of Liberty safeguards.
- Relevant specialist clinical/professional courses/training.
- Application of evidenced based interventions relevant to the service.
- Evidence of working with people who have experienced a mental health problem.
- Experience of working in a multi-Disciplinary environment.
- Experience of working in a secondary care community mental health service.

Job summary
30 hours per week - permanent.
There is an exciting opportunity to join, the
Mental Health Assessment and Intervention Team. The service was set to provide
high quality care for people experiencing mental health problems in Plymouth
and has proved a success across the city. The service will offer evidence-based
treatment pathways for people that require more support than can be offered in
Primary Care Services and voluntary agencies within the community. The role
will develop more effective links with primary care and third sector agencies
to support those people.
Main duties of the job
We are seeking to recruit experienced and
flexible with excellent networking skills to join the team. This post will
offer a challenging and exciting opportunity to gain experience of working
across Livewell's Core mental Health Service, working across a variety of
settings such as GP surgeries, and Community hubs developing relationships with
Primary Care, partner agencies and secondary mental health services. You will
have the opportunity to deliver evidence-based recovery focussed mental health
interventions.
