Role summary
This role is for a passionate and caring district nurse to lead the Unplanned Care Team Triage Hub. You will oversee the provision of high-quality holistic care for patients in their homes, preventing unnecessary hospital admissions and facilitating early discharges. The role involves clinical leadership, managerial support, and expert guidance to a team of nurses and healthcare support workers, operating on a rotational basis within a seven-day service.
Key responsibilities
- Provide clinical leadership, managerial and supervisory support for the Triage Hub and staff.
- Work in a clinical role, providing expert guidance and clinical knowledge to staff members.
- Lead the Triage Hub daily, implementing and leading robust HUDDLES.
- Ensure effective day-to-day management and support to the team.
- Work closely with planned and unplanned services, participating in change management and service development.
Essential criteria
- RGN
- Completion of a recognised leadership course
- Open, participative, and supportive leadership and management style.
- Ability to define and develop effective systems of service delivery.
Desirable criteria
- District Nurse SPQ
- Evidence of further formal management study or qualification
