Role summary
This role involves working as a Hospital at Home Team Nurse, providing skilled nursing services to patients discharged from hospital who continue treatment at home. The post holder will educate patients/carers and multidisciplinary teams, fostering relationships with primary and secondary care colleagues to facilitate integrated care and early discharge. The aim is to prevent inappropriate hospital re-admissions and support patients safely outside the acute setting.
Key responsibilities
- Assess care needs, deliver and implement management plans for individual patients using evidence-based care.
- Provide high-standard care for patients assessed as safe and medically suitable to remain at home.
- Work with specific patient groups within agreed pathways in Primary and Secondary Care, managing their ongoing care and safety.
- Assess identified or referred patients for the Navigation or Hospital at Home Service as part of admission avoidance.
- Participate in effective and safe discharge planning for inpatients/outpatients, promoting smooth transition to the Hospital at Home Team or other community services.
- Support patients in their home environment as part of the Hospital at Home Virtual Ward, ensuring safe recovery.
Essential criteria
- NMC PIN
- Experience working as Band 6 within an acute specialism or Band 5 with the relevant nursing experience
- Some managerial experience
- Evidence working in acute setting
- Experience of supervising junior staff
- Understanding of Clinical Governance
- Knowledge of risk management
- Audit and research and evidence based practice
- Knowledge of National and Local Policy agenda's relating to post
Desirable criteria
- Clinical experience of working with community services
- Training of history taking and knowledge of coagulation

Job summary
The post holder is identified as a Hospital at Home Team Nurse, working in conjunction with the current H@H teams, providing skilled nursing services for patients who are suitable for discharge and to continue their treatment at home. The Hospital at Home service offers a seamless patient journey across Primary and Secondary Care. The post holder will provide education and training for patients/ carers and members of the multidisciplinary team across boundaries; share nursing knowledge and skills to support and advice as needed. The post holder will develop effective working relationships within Primary and Secondary Carecolleagues, GP's and other Primary Health Care Teams to facilitate integrated care and enable early supported discharge, prevent inappropriate re-admission to hospital. Support and participate in the development of integrated pathways and care packages which enable patients to be safely cared for outside of the acute hospital setting.
