Role summary
This role is for a Lead Occupational Therapist within the Care Transfer Hub, focusing on coordinating care for patients discharged from hospital. The hub ensures safe discharges by linking community services and providing support for onward care. The post holder will lead in supporting complex patient discharges related to function and cognition, working closely with acute and community wards. This is a fixed-term contract until March 2027, due to external funding.
Key responsibilities
- Accountable for delivering high-quality therapy care within a defined clinical area or pathway.
- Provide expert clinical practice with advanced clinical reasoning and support to staff.
- Offer clinical leadership to develop staff within the team.
- Lead the evaluation and development of clinical practice within the specialism, ensuring compliance with National Frameworks.
- Assist and support Clinical Service Managers and Lead Therapists in improving AHP services within the Trust.
Essential criteria
- BSc in Occupational Therapy or equivalent
- Current HCPC Registration
- Postgraduate Masters level module in relevant subject to this post or equivalent knowledge and skills accumulated through formal training and experiential learning
- Evidence of continuous personal and professional development
- Current Knowledge of evidence -base around discharge and intermediate care
- Up to date NHS policy initiatives, changes and developments
- Extensive experience as a registered OT
- Experience of Operational line Management including appraisal
- OT Clinical Educator
- Participation in Clinical Audit & Practice Development
- Experience of managing change in complex operational environments
- Experience of the management of complex patients along the discharge and intermediate care pathways.
- Experience of working with the wider health and social care system
- Excellent team working skills and ability to work collaboratively
- Excellent interpersonal and communication skills - to include written, oral and presentation
- Excellent organisational skills, ability to act on own initiative, to prioritise work and manage multiple tasks for self and other.
- Ability to analyse complex problems, formulate solutions and implement sustainable change.
- Confident in working with a wide range of senior professionals
- Ability to motivate and engage wide range of individuals
- IT literate - interpretation of data and development /analysis of reports
- Understanding of stress and ability to help others to cope under pressure.

Job summary
Discharging an individual on to the right care pathway when they no longer need to remain in a hospital or community bed requires a whole system approach. Every local health and care system has a transfer of care hub where (physically and virtually) relevant community services across sectors (e.g. health, social care, housing, voluntary sector) are linked together.
The hub coordinates care for people who require care and support after discharge from hospital, and any support for unpaid carers providing care. This covers any patient discharged from hospital or community beds regardless of the area they are resident in. The hub is staffed by a small team, dedicated to ensuring people are discharged from hospital on the right pathways, with the right discharge information, and that they get the right onward care and support if needed.
The hub supports safe discharges through close working with the acute and community wards, quality assurance of information and practical support, including proactive early identification of those who may become ready for discharge, supporting planning as early as possible following admission.
